Medical and Personal Information

Please type “n/a” if there are no known allergies at the time of registration. Do include details about level of allergic reaction where appropriate. Please note that all families will be notified of any anaphylactic allergies or similar (without identifying the child’s name) one week prior to camp for meal planning and awareness.
Please type “n/a” if there are no medications being taken or anticipated at the time of registration. Do include details about level of care for administration and storing of any medicines. Please note that CDCC staff may not be authorized to administer all types of medicines or manage all scheduling regiments.

Final Submission

Upon pushing “SUBMIT CAMP REGISTRATION” – A new window will open to make all required online credit card payments. A copy of this registration and payment receipt will be emailed to the primary parent/guardian email address. Please ensure you have received an emailed copy of your registration to confirm your registration. Spaces are limited, and application forms will not be accepted by the system if camp capacity has been reached. One form is required per camper. A reminder that campers may only register for one week of CDCC Arts Camp in 2026, and the first application only will be considered if two applications are received.
