Request for Replacement/Display Diploma
This form will guide you through the process of ordering a replacement or display diploma.
A replacement diploma may be requested if your original diploma was lost, damaged, destroyed, and/or where your legal name has changed since the diploma was issued
- Before we can process your request, you must provide all requested documents, complete payment, and in cases of a name change return the original diploma if it has not been lost.
- Note: If your diploma has been lost/damaged/destroyed, we will require you to obtain and scan a notarized Statutory Declaration (see here for more information).
- We will also require you to scan a copy (front/back) of valid government-issued photo identification.
- In addition, if your legal name has changed, we will require you to scan a copy (front/back) of the appropriate name change document.
A display diploma may be requested if you wish to obtain a larger-format version of your diploma, suitable for display/framing.
